Answer calmly. Choose the number that best describes how you felt in the last two weeks. There are no right or wrong answers.

1. Have you felt sad or empty almost all day?

Not just "bad mood". It's that feeling that nothing satisfies you, not even the things that used to. If it lasts for days, it's an important sign.

2. Have you lost interest in things you used to enjoy?

Hobbies, going out, eating something nice. Sometimes you stay home without wanting to see anyone, even if you used to enjoy those plans. Recognizing this helps.

3. Do you have trouble sleeping or sleep too much, almost daily?

Sleep changes with mood. Not sleeping well, or sleeping too much, can be your body's way of signaling that something is wrong.

4. Do you feel tired, lacking energy, even without having made a great effort?

Not just sleepiness. It's fatigue that doesn't go away with rest, as if you were carrying an invisible weight all day.

5. Have you found it hard to concentrate on simple things (reading, watching a movie, doing errands)?

Sometimes exhaustion doesn't come from working or walking a lot, but from carrying the weight of the day. That tiredness that doesn't go away even with sleep also needs to be heard.

6. Have you thought that life isn't worth living or had thoughts of wanting to disappear?

Prolonged sadness often comes with a very harsh inner voice. Recognizing that voice is important — not to stay there, but to know that you need help to silence it.
